Description

A penis erection occurs in response to physical or psychological stimulation.

This stimulation causes certain smooth muscles to relax, increasing blood flow to spongy tissues in the penis, as a result, the blood-filled penis becomes erect.

After stimulation finishes, the blood flows outwards and the penis returns to its flaccid state.

Priapism is an uncommon condition where there is a prolonged & painful erection of the penis which isn’t caused by sexual stimulation.

If not treated quickly, it can cause permanent damage to the penis and prompt treatment is needed to prevent tissue damage that can lead to inability to get or maintain an erection.

It most commonly affects men in their 30s and older but can begin in children too if they suffer from sickle cell disease.

There are two main types of this condition:

  1. Ischemic or low-flow priapism – a more common type where the blood is not being able to leave the penis. Blood is trapped because it cannot flow out of the veins of the penis or there is a problem with the contraction of smooth muscles within the erectile tissue of the penis.
  2. Non-ischaemic priapism also known as high-flow priapism, occurs when blood flow through the arteries of the penis isn’t working properly – that often occurs due to trauma.

 Common Signs & Symptoms Ischemic priapism

  • Erection lasting more than four hours that is unrelated to sexual interest or stimulation.
  • Fully rigid penile shaft, but the glans (tip of penis) is soft
  • Progressively worsening penile pain

Signs and symptoms of Nonischaemic priapism include:

  • Erection lasting more than four hours or unrelated to sexual interest or stimulation
  • Erect but not fully rigid penile shaft
  • Usually not painful

Possible Causes & Risk Factors

In most cases due to sickle cell disease that can block the blood vessels in the penis.

Other causes may include:

  • Penis cancer
  • Anti-coagulant or blood-thinning medicines
  • Antidepressants or drugs to treat ADHD, also use of cannabis, marijuana and cocaine
  • Certain medicines for high blood pressure, thalassaemia and leukaemia etc
  • Metabolic condition like gout or amyloidosis
  • Neurogenic issues like a spinal cord injury or syphilis
  • Certain treatments for erectile dysfunction – especially when injected directly into the penis.
  • Hormones such as testosterone or gonadotropin-releasing hormone.
  • Alcohol and drug use
