Epilepsy is a brain disorder that causes recurrent episodes called seizures, as an electrical storm in the brain leading to abnormal movements.

If the seizure occurs in a localized part of the brain, it is called a partial seizure and if it affects much of brain, it is called a generalized seizure.

Epilepsy can affect people of all genders, races, ethnic backgrounds and ages.

There are different types of epilepsy:

  • Petit-mal epilepsy or an absence seizure cause blanking out or staring into space for a few seconds and mostly common in children. They are brief lapses in consciousness and can involve purposeless movements like jerking or rapid blinking.
  • Tonic-clonic or grand mal seizures can lose consciousness, making the patient fall to the ground, have muscle jerks or spasms, and cry out.
  • Frontal lobe epilepsy or brief focal seizures can cause muscle weakness, unusual movements and some loss of awareness
  • Temporal lobe epilepsy or focal seizures are often connected with auras of nausea, emotions, or unusual smell or taste
  • Neocortical epilepsy: Seizures that commence from the cerebral cortex, or outer layer, and can be focal or generalized
  • Photosensitive epilepsy that are triggered by flashing lights
  • Atonic seizures or drop seizures, cause muscles to go limp and can result in a fall to the floor
  • Childhood occipital visual epilepsy: A self-limited epilepsy syndrome that begins in childhood.

Common causes of epilepsy in adults.

In many instances the cause is unknown, however it could be due to:

  • Genetic inheritance
  • Interruption in the electrical activity in the brain
  • Hypoglycaemia – low blood glucose levels
  • Lead poisoning.
  • Brain damage at birth
  • Infection & head injury
  • Alcoholism & drug addiction
  • Stroke and other vascular diseases.
  • Brain infections such as meningitis – inflammation in the brain or spinal cord, can increase risk of epilepsy.
  • Dementia

Common causes in Children

  • Due to high temperature
  • Because of an infection e.g. throat or ear etc.

Common symptoms of epilepsy in adults

  • Loss of consciousness that can continue for seconds to minutes.
  • Tongue biting
  • Rhythmic twitching of extremities or face.
  • An “aura” is felt where the patient knows the convulsions are coming.
  • Severe spasms & jerking of the hands & legs with rolling up of eyeballs & frothing at mouth.
  • After convulsions there is a period of weakness or paralysis.
  • Loss of memory for the event.
  • Uncontrollable muscles movements – convulsions
  • Sudden loss of responsiveness
  • Rigid body with an arching back
  • Noisy difficult breathing
  • Loss of bladder and bowel control
  • Afterwards confusion, tiredness and falling into a deep sleep

Common symptoms – children

  • Vigorous shaking with clenched fists and an arched back
  • Signs of fever – hot, flushed skin, and sweating
  • Twitching of face, squinting, fixed or upturned eyes
  • Breath holding with a red, puffy face and nec
  • Drooling saliva from mouth
  • Vomiting
  • Loss of control of their bowel or bladder
  • Partial or full loss of responsiveness

Common Seizure triggers

Triggers don’t cause epilepsy, but they may trigger seizures in people who have epilepsy.

Possible triggers may include include:

  • Skipping doses of antiseizure medicines or taking more than prescribed.
  • Lack of sleep.
  • Hormone changes during the menstrual cycle.
  • Flashing lights.
  • Illicit drug use.
  • Skipped meals.

Suggested First aid approach

  • Don’t restrain or move the patient.
  • Protect them from hurting themselves.
  • Clear away any potentially dangerous objects, like hot drinks or sharp objects.
  • Note the time when the seizure started and how long it lasts.
  • Protect their head by placing something soft underneath.
  • Loosen any clothing around their neck.
  • Open their airway and check breathing, if they fall into sleep.
  • Provide them fresh air by opening a door or window (Don’t cool them down too much!)
  • If they’re breathing, put them in the recovery position.
  • If they stop breathing at any point, seizure last more than 5 minutes, and unresponsive for more than 10 minutes, Call for emergency help.
